Pain in the ball of the foot (metatarsalgia) most often comes from overloading the metatarsal heads, but specific causes include Morton's neuroma, plantar plate tears, stress fractures, and sesamoid problems. The location and character of the pain help narrow it down.
Burning or numbness between the toes
Often a Morton's neuroma, usually between the third and fourth toes, and worse in tight shoes.
Pain under the second toe
Frequently a plantar plate injury, especially if the toe is starting to drift or lift.
Pain under the big toe joint
May involve the sesamoid bones or arthritis in the big toe joint.
Swelling on top of the foot
After an increase in activity, this raises concern for a stress fracture.
What helps
Cushioned shoes with a rocker sole, a metatarsal pad, and avoiding high heels are good first steps. Pain lasting more than two weeks deserves an exam.
