Before your visit
- Complete the new-patient forms if you can, and bring them with you.
- Bring your insurance card, photo ID, and a list of your medications.
- Bring any recent X-rays, MRI reports, or operative reports related to your foot or ankle.
- Wear or bring the shoes you use most — they often help with the diagnosis.

Coverage varies by plan. Please call before your visit so we can verify your benefits.
Urgent symptoms
For spreading redness, fever with a foot wound, a cold, pale, or numb foot, or a severe injury with deformity, go to the nearest emergency department.
Your privacy
Please do not send symptoms, photos, or other medical information by email or through social media. We will collect what we need securely when you call or visit.
